RE: MEDICAL EXAMINATION FOR NEW ZEALAND VISA

PLEASE READ ALL THE RELEVANT INFORMATION FROM NEW ZEALAND IMMIGRATION SERVICE. READ FORM INZ 1007 – PAGES 1 – 4

You need to bring the following with you when you attend for the medical:

  • Medical Form, INZ 1007. Please complete Section A and B.
  • Consultant’s reports if any medical problems now or in the past.
  • Medications and spectacles (if any).
  • Current passport (plus passport used on application, if different).
  • 3 passport photographs.
  • A full physical examination is required. Female candidates who wish to see a female doctor should let us know when they are making the appointment.
  • Payment for the medical: €250 incl. v.a.t. per adult and approximately €150 incl. v.a.t. per child.
  • Check with Euromedic for X-Ray costs

For women: Women should not attend this medical examination during menstruation as blood will taint the urinalysis.

Blood Tests: Blood tests are required for diabetes, hepatitis B & C, HIV , syphilis and kidney function.

RE: CHEST X-RAY
The panel doctor will give you a referral note for the chest x-ray. You may attend Euromedic’s X-ray Department immediately after your medical examination. Euromedic is approximately a five-minute drive or fifteen-minute walk from our surgery. You do not need to make an appointment for the chest x-ray but please note Euromedic closes at 5.30pm each day.

Directions: We are located on the Douglas Road, near Douglas Village, next to the Briar Rose Pub. The surgery is over the shops in the Ardfallen Mall. Go around to the back of the building and up the stairs. Euromedic is in the centre of Cork City in the Elysian Tower beside the City Hall.

Please attend on time for your appointment. If you miss your allocated time we may not be able to see you.
