Download all the details you require for a medical for Canadian visas here. Note you need to have all these details before visiting the clinic. We cannot complete the medical if we are missing any of the information.

RE: MEDICAL EXAMINATION FOR CANADIAN VISA

PLEASE READ ALL THE RELEVANT INFORMATION FROM THE CITIZENSHIP & IMMIGRATION CANADA

You need to bring the following with you when you attend for the medical:

Your form, IMM 1017, “Medical Report Section A, Client Biodata and Summary”.

Your ID number (IME/UMI) to allow us to access your case on-line.

(If you have not already lodged an application with CIC , we may be able to do an “upfront medical” . This involves us creating a new CIC  casefile for you and therefore can only be used if you dont have a pre-existing file )

 

  • Consultant’s reports if any medical problems now or in the past.
  • Prescription medications and spectacles (if any).
  • Current passport (plus passport used on application, if different).
  • A full physical examination is required so female candidates who wish to see a female doctor should let us know when they are making the appointment.

Payment for the medical: €250 incl. v.a.t. per adult and €150 incl. v.a.t. per child. If further visits are required, further fees will apply.

Check with Euromedic re fee for xray.

For women: Women should not attend this medical examination during menstruation as blood will taint the urinalysis.

Blood Tests: Blood tests are required for HIV and syphilis. These will be taken at the time of the examination.

RE: CHEST X-RAY
The panel doctor will give you a referral note for the chest x-ray. You need to make an appointment with Euromedic’s X-ray Department preferably after your visit to us.  Telephone (021) 431 9995

Euromedic is approximately a five-minute drive or fifteen-minute walk from our surgery.

Directions: We are located on the Douglas Road, near Douglas Village, next to the Briar Rose Pub. The surgery is over the shops in the Ardfallen Mall. Go around to the back of the building and up the stairs. Euromedic is in the centre of Cork City in the Elysian Tower beside the City Hall.

Please attend on time for your appointment. If you miss your allocated time we may not be able to see you.
